베드나르 아프타 (Bednar aphthae)

- 젖병으로 수유와 연관된 mechanical trauma

- small, shallow, symmetrical ulcers in posterior hard palate or soft palate (midline에 생기는 경우도?) 

- no systemic sx such as fever 

 

- 생후 초기에서 4개월사이에 가장 흔히 발생

 

-DDx 

: herpetic gingivostomatitis, herpangina, HFM disease

-> 발열이 없는 점과 궤양의 위치에 의해 구분 가능 

-> HSV는 입술, 잇몸, 혀도 흔히 침범

* symmetrical morphology, characteristic location in palate, 4개월 미만에서 발생, 젖병수유 -> Bednar aphthae를 시사

 

-Tx 

: Horizontal position으로 feeding하지말고 semi-seated position으로 수유하도록 교육 

: 젖병의 꼭지 구멍을 더 크게 

: 더 부드러운 재질의 젖병을 사용 

: 보통 1달이내에 특별한 후유증 없이 회복
