특별한 기저 질환이 없던 환아가 failure to thrive, gastroenteritis, asthma, viral infection 등으로 내원하여 혈액검사를 시행했을 때 ALP (alkaline phosphatase) 수치가 정상보다 높게 발견되는 경우가 있다. 대부분의 환자들에서 이런 현상은 일시적인 상승으로 생각되며 거의 4개월 뒤 재검시 정상화 되는 경우가 대부분이다.
ALP isoenzymes: bone(85%), liver(15%), intestine, kidney, placenta, white blood cells, endothelium
-> enzyme의 역할: hydrolyze organic phosphate esters at an alkaline pH to form phosphate and an organic radical
- Infant와 children에서 ALP는 높은 osteoblastic activity 때문에 어른보다 보통 높다.
- 영아에서 성인 정상 수치의 3배가량 높게 나오다가 2세경 성인 정상수치의 2배가량으로 떨어진다
- 사춘기때도 성인 정상 수치의 2-3배 가량 올랐다가 1-2년 뒤 정상 수치로 떨어진다.
Diagnostic Criteria
- ALP가 정상의 3-50배 가량 상승
- 5세 미만
- Variable unrelated Sx ranging from normal to severe gastrointestinal or neurologic impairment
- No clinical evidence of bone or liver disease
- No laboratory evidence of bone or liver disease except for the marked elevation of serum ALP
- Serum ALP isoenzyme elevated in both bone and the liver
- A normalization of serum ALP levels within 4 months of onset
-거의 대부분의 케이스가 5세 미만이지만, 25% 정도에서는 5세 이상, 그리고 성인에서도 가끔 발견되기도 한다.
- 20% 정도에서는 수치 정상화가 4개월 이상 걸리는 경우도 있었다.
- 13% 정도의 환자에서는 기존의 만성질환 (leukemia, lymphoma, liver disease, rickets, metabolic osseous activity, kideny or liver transplant)이 있었던 경우였다.
- 대부분의 THP 환자들은 liver나 bone-isoenzyme이 상승하는 것으로 보였으나 일부 케이스들에서는 intestinal isoenzyme이 상승하는 경우도 있었다. (Gastroenteritis와의 연관성)
Differential Diagnosis
- Hepatobiliary: cholestasis, intrinsic liver disease, malignancy w/ liver metastases, renal disease, bowel disease (bowel perforation, infarction, inflammatory bowel disease, pancreatitis, malabsorption). 짙은 색의 소변, 가려움, 멍, 지방변을 보는 환자에서 가능성이 있다.
- Bone: rickets, osteomalacia, healing fracture
- Vitamin D deficiency도 Ca, P, PTH 이상 소견이 있는 환아라면 고려해볼수 있으며, 햇빛 노출이 적은 환자, 장기간 모유수유만 하는 환자, vitamin D가 보강되지 않은 분유를 섭취하는 환자에서 고려해보아야 한다.
